Between 1991 and 1995, she was Deputy in the Regional Assembly of Murcia Cartagena as well as deputy spokeswoman for the People' Party.

Pilar Barreiro Alvarez became the 17 June 1995 the first woman mayor in the history of Cartagena to obtain the majority support of citizens in the candidate of the Popular Party, which won 15 of the 27 councilors of the municipal corporation. In 1999, 2003, 2007 and 2011 revalidated such absolute majority.  
In January 2008, she was appointed by the People's Party of the Region of Murcia as chief candidate for Congress in the general election. She was elected as Deputy with 61.43% of the vote.
